Once Azum Medical sends your Patient Portal invitation, you will receive an email with instructions to activate your account.
Open Your Invitation Email
Look for an email inviting you to the Azum Medical Patient Portal. Open the email and click Get Started.
Create Your Account
You will be taken to the Patient Portal account setup page. Create a secure password and enter your date of birth. Your email address should already be entered automatically from the invitation.
Activate Your Portal
Click Create Account. Once your account is created, you will be taken directly to your Patient Portal dashboard.
Once activated, you're ready to get started.
